Every decision is written to a hash-chained ledger. A removed or edited entry breaks every link after it.

The record is designed to be handed to someone who does not trust you, and to survive that.

Governance here is a floor, not a leash. It constrains what may be said; it does not pretend to be the whole of judgement.

Blue is the accountability grade: it describes what survives after the answer is given.
